MPD doesn't know if mpc or PHP connects to it, so this can hardly be a difference. I rather think your PHP code is buggy, but I can't tell without seeing it.

Not quite. If you want to have MP3 files, then you should use LAME, because only LAME adds metadata to the files which allow MPD to do gapless MP3 playback. But that doesn't imply that you must use MP3 for gapless playback. Other codecs are always gapless (Vorbis!), and don't need this kind of metad...

It's a pity that Debian-based distributions (e.g. Ubuntu and Raspbian) ship with outdated and discontinued MPD versions. Their refusal to provide bug fixes to their users creates a lot of sorrow. These distributions need to hear more loud complaints from their users.

Your MPD version is old and unsupported.
But my random guess is that the upgrade broke the NFS client, and now MPD times out while waiting for data from the NFS mount.
If you need further help, first upgrade to a supported version (i.e. 0.21.13 or git master).

So you have two devices: one for analog output (to headphones) and one for HDMI output. Your MPD config uses analog output only; nothing should be on the HDMI output. Paste the output of "aplay -l".
After that, paste the output of "cat /proc/asound/card0/pcm0p/sub*/[sh]w_params" while MPD plays (being too silent), and again while a different software plays which has "good" volume.

Try forcing 16 bit output by adding format "*:16:*" .. to the ALSA output section. This reduces quality by discarding a few bits of precision, but I've seen some DACs misinterpret "unusual" bit depths (i.e. not 16 bit), so this may work around such a DAC bug. What does "alsa is not supported" mean? MPD can control any ALSA mixer, just like the "amixer" program. If amixer can control it, then it's "alsa supported".

As long as your driver fails to properly report those xruns to userspace, there's little we can do about the situation. Maybe you can reduce the number of xruns by using some "magic" numbers for those buffer settings, but it doesn't change the fact that your driver (or some other entity behind MPD) ...
